Insurance Certificate Process
In order to complete the proces of your affiliate registration, you must have your insurance agent, broker or company send to us an original certificate of additional insured. Our name and address must appear as; USA Bus Charter, 14175 W. Indian School Road, Suite B4-146, Goodyear, Arizona 85395. You may fax a temporary certificate but we "must" have an original on file in order to start placing charters with your company.
